pdf forms from makinghomeaffordable.gov

I have a Dell laptop and I am running Windows 8 with Adobe Reader 11.1 but when I go to the makinghomeaffordable.gov web page and ask for the form to apply for mortgage assistance I get stuck on a page that never changes even if I follow the update instructions and restart my computer. The page looks like this -
