Bar code not working

I have a pdf form with a bar code, which collects all the form contents from page one. But when i edit any textbox or ant check box, the barcode crashes and only a grey box shows. Why this is happening. I searched it and some guys says that, that form has to extended form live cycle by reader extension. Is that the exact way to solve it? Here i attached the pdf form
