interactive PDF buttons not working in acrobat X

I have exported an interactive PDF from indesign CS5.5.

I am trying to view it with acrobat pro X 10.1.6

 

The file has a table of contents with a 'contents' buttons to hide and show the table. It works in the Preview within indd, however when I export it as an interactive PDF two things happen:

 

The 'Contents' button is visible as it should be. When I click it, the table doesn't appear as it is supposed to, yet the contents button disappears (as it is supposed to). Nothing else happens. I can't get the table to reveal or the contents button to com eback. I set up the interactive table on a master page, so it should appear on every page. When I scroll throug the pages in the PDF the contents button doesn't appear here either once it has hidden itself.

 

Another odd thing that may be related; I get a purple bar across the top of the page that asks me to fill in a form, or if I am the administrator, to distribute the form using the Forms Panel.

I haven't added any functionality like this into the document.

the contents button is the small plus mark. This is the view from the preview in indd. The buttons all work, the functions are correct.

This is the view in PDF of the purple bar that comes up. I can't get rid of it.
